Appointment Booking Request

Complete the form below to request a booking for your pet.  Dr. Sarahbeth Barlas, DVM will personally review this information prior to your appointment, and these questions are key to directing medical care.  Please be as detailed as possible to help us better serve you! 

Forms are reviewed by a member of the HCVS staff.  After completing this Appointment Request Form, we will contact you within 48 hours M-F to schedule an appointment. 


Booking requests are not a guarantee of service. We will contact you to schedule an appointment and send a prepayment invoice for Exam + Travel to confirm your booking.
